How should a healthcare practice track revenue by provider and by payer?

Tracking revenue by provider and by payer starts with how your accounting software is configured. In QuickBooks Online, classes work well for providers. Create a class for each physician, nurse practitioner, or therapist generating revenue. Every payment posting gets tagged to the provider who performed the service. This lets you run profit and loss reports filtered by provider so you can see who is producing what.

For payer tracking, set up your income accounts or customer records to distinguish between payer types. At minimum you want categories for Medicare, Medicaid, each major commercial insurer, and self-pay. Some practices go further and break out individual plans within a payer. The level of detail depends on your payer mix and how granular you need your reporting to be.

The tricky part is not the setup. It is how payments get posted. Insurance rarely pays the full billed amount. A $200 charge might result in a $140 payment and a $60 contractual adjustment. If you only track what was billed, your revenue numbers are inflated and meaningless. You need to record the payment received, the contractual adjustment, and any patient responsibility separately. That way your reports reflect actual collected revenue by payer, which is the number that matters for decision-making.

Run your practice management system reports alongside your accounting reports and reconcile them monthly. Your PM system tracks claims and payments at the encounter level. Your accounting software tracks the money that actually hits your bank account. When these two systems agree, you have confidence in your numbers. When they don’t, you have a problem to investigate before it gets bigger.

The insights from this tracking are worth the effort. You can identify which providers generate the most revenue relative to their compensation and overhead. You can see which payers reimburse well and which ones consistently underpay or delay. That data gives you leverage when renegotiating payer contracts and helps you make informed decisions about which insurance panels to stay on.

Practices that track revenue this way also catch problems faster. If collections from a specific payer suddenly drop, you can investigate whether it is a coding issue, a credentialing lapse for a particular provider, or a change in the payer’s fee schedule. Without provider and payer level tracking, these problems hide inside your total revenue number until cash flow gets tight.

If your books are currently lumping all revenue together without distinguishing providers or payers, getting this structure in place is a priority.
